19 Dec Thick descriptions of promising integrated care programmes online!

We’re happy to announce that the thick description reports we have been working on the past year are now available on the SELFIE website, under the ‘Publications‘ tab. An executive summary of all programmes is also available.

These reports describe promising integrated care programmes for persons with multi-morbidity in each of the 8 SELFIE partner countries. A variety of programmes is described:

The Netherlands: Two programmes targeting frail elderly, and one programme targeting persons with problems in multiple life domains.

Austria: Two programmes targeting frail elderly, and one programme targeting persons with problems in multiple life domains.

Croatia: A palliative care programme and a programme targeting frail elderly.

Germany: A population health management programme and a programme targeting frail elderly.

Hungary: A palliative care programme and an oncological programme.

Norway: A programme targeting persons with problems in multiple life domains and a palliative care programme.

Spain: A population health management programme and a programme targeting frail elderly.

UK: Two population health management programmes.
