Transferability to Central- and Eastern Europe (CEE)

MAIN OBJECTIVES

The necessity for a multi-disciplinary, holistic approach to facilitate health of persons with multi-morbidity has been recognized in CEE countries. However, CEE countries have limited human resources and financial capacities to develop ICC models. Therefore, WP7 aims to enable the transfer of knowledge and tools generated in previous WPs to CEE countries.

APPROACH

  1. To conduct a qualitative survey among CEE stakeholders on the implementation barriers and facilitators of ICC models, financial incentives, and performance monitoring.
  2. To develop the implementation strategy of ICC models, financial incentives, and performance monitoring from WP6 to partner countries Hungary and Croatia.
  3. To tailor the implementation strategy following from WP6 to other CEE countries.
  4. To organize and host the Transferability Conference for CEE stakeholders, focusing on the transferability of evidence based ICC models, financial incentives, and performance monitoring from higher income European countries to CEE.
